i've read many times of players being suspended for minors, substance abuse, discipline issues on and off the ice. beyond multiple substance abuse issues are there any reasons that will get someone kicked off a team.

It depends on the coach. Some are obviously more tolerant than others. MSHL rules to be followed at minimum. After a certain number of offenses you may loose your eligibility.
